Toxic work environment
I worked in the Colorado area for LHC group and would strongly advise you to work elsewhere. The new management that took over about a year ago has successfully driven out many good nurses and therapists simply because they stated disagreements with her. No one feels comfortable giving any sort of feedback to this director because she acts in retaliation. I have seen this happen to a good coworker of mine. After this person disagreed with the director, the next day her schedule was cleared and she was told she was being let go because of low caseload which wasn't the case at all!! She is manipulative, cunning and hateful with narcissistic tendencies. She is mean. I've never worked for someone like this and it has been toxic. I've been asked to fudge my Oasis scores on patients to make it look like they've "improved" at the end of episode. If I don't do it, I'm called into the office and threatened disciplinary action. My skills as a clinician have been questioned and I've been asked to provide certain services to patients that I'm not qualified to do or don't feel comfortable doing. There is no repercussions for this director's actions, corporate/higher management do not care based on their lack of follow through with any complaints given of her, in fact she moved from clinical director to executive director. I had no idea that having an employee retention rate of less than 40% would get you promoted! Prior to this director, employee retention was 90%. Now this executive dire

Decent Pay but lots of driving, Stressful, Unorganized Scheduling, and Understaffed
As an LPN, I loved the job itself... seeing patients in their home and having the flexibility to schedule my own day with the patients. The pay was pretty good for an LPN in our area. The actual scheduling department in the office is unorganized so i basically did my own scheduling to avoid spending more time in my car than on patient care. They really don't care how much you have to drive. Agency is extremely understaffed and you are expected to be available and go above and beyond in any area they need you regardless if youve already completed your productivity (or expected work). If youre lucky enough to have an understanding team leader, like i was, they will help ease some of your stress by helping call mds and obtain orders for you but thats the exception here, not the rule. Most of the time, you are basically on your own. You bring alot of work home with you due to lots of documentation unable to complete in the home because you have to maintain a certain time limit in the home and stay on a strict schedule to be able to get all your work done... and then you still have to schedule your next day and call all your patients the night before. Not to mention, you use your personal cell phone so patients and families often call you at all times of the day/night as well as doctors offices. The work never seems to end. A typical day working for Caretenders Home Health would start around 8 am at the office where I would stop to turn in my previous days paperwork, and pick up supplies that I would need for the current day. While in the office I would let the office staff know of my plans for the day and let them know if I was ok with my patient load. It would be at this time that I might let them know if I were in need of additional patients or if I was unable to take on more patients. I may pick up a new referral while in the office. I may paricipate in a case conference with other disciplines envolved in my patients case such as : PT, OT, HHA, MSW. I may contact MD's for required orders. There are numerous resources available at the office needed to provide optimal patient care. Finally I would leave the office and get on the road to my 1st patient. I would see a variety of patients including among others: post-op orthopedic or cardiac, COPD, new diabetics, patients in need of extensive wound care, trach patients and patients requiring IV infusions. Often I may need to do blood draws for a variety of labs or PT/INR's. Always my visits included head to toe assessments, med teaching and answering patient concerns. Typically I would see 6-8 patients a day. Ocassaionally that number could go up to 10-12 patients a day. If you have a license and a warm body, congratulations you are hired! Company offers Greet insurance benefits but you’ll never have time or be allowed time off to use it, Turnover is insane no one stays with this group. Lpns will be scheduled 10 sometimes 12 patients a day. Which could be done if patients were not so far apart. You don’t get paid for that time either. Just your mileage which is less than average RNs or LPNs have no idea where they will be day to day. You can’t follow a patient because the scheduler does not have a clue. You may end up in Maryville, Lonesdale, Karns, Turkey creek and Oakridge on the same day. The clinicians are crossing paths all day. You will drive 40 miles to see a patient that’s 1 mile from another clinicians patient and that clinician will be doing the same in an area you have patients. If your an RN you will do 4 or more SOCs a day and are expected to be done charting within 24 hours. You literally will work from early morning til your eyes slam shut finishing your charting at midnight. Working M-F then on call e/o weekend mandatory. You’ll be offered to pick a day off but it will need to be approved first and it won’t be because someone quit so you end up working 13 days straight. Management is cut throat They don’t care if you stay or go. You will never get paid right. Checks are always wrong. To Lhc’s benefit of course. You can’t call payroll. They really don't care who you are. You are a warm body to fill Medicare numbers. Ever since merger went through old CEO went to the new CEO and put such unreasonable expectations on staff to "impress" that they are forgetting what patient care is all about. Why would anyone put themselves on a rat wheel when end result is you are never doing enough for this company?? They demand all these meetings on top of already high expectations out in the field and bottom line is you may have done good yesterday but you need to do double that today. And if you don't? You aren't investing yourself and must not care enough, or be applying yourself. What? They will find any way to get out of giving you credit for any of your efforts. And when you don't follow the grain of processes for any reason at all, you better believe you will be on your way out. It's a disgusting culture it's turned into where they claim it's all so empowering and a great vision but it's full of constant micromanagement and defense of your efforts. They will never see it that way because they all drink the company cool aid but the fact is they don't even understand they are asking way too much of their employees for very if any recognition or reward. Commission is a joke. Your number will always increase for expectation month over month regardless of tenure and you have to hit at least 5 over that to even make any money at all in commission after fighting for credit for every single admission. My favorite job has been home health social work.
I have been a social worker at LHC sonce 2011. I see patients and their caregivers. I help them get resources and benefits they might not have known about. I explain their options to patients. Once they decide what path they want to take, I help them navigate Medicaid and VA benefits, finding caregivers or finding placement. I spend time supporting caregivers and getting them respite or compensation. I also help caregivers understand better ways to communicate with dementia patients. I help both patients and caregivers cope with life changes and the emotions that come up. My nurses, phyiscal therapists, and management treat me as a resource they can call if they have questions about family dynamics, meals on wheels, POA, caregivers etc. I support staff when they are dealing with difficult cases. I feel respected and appriciated in my job. One of the things I love about my job is the autonomy I have. I set my schedule to see patients. I decide a plan of action. I decide how many vvisits a patient needs. If I see a patient once and they are not ready to take any action Iask staff to call me back in when the patient is ready. The team I work with is the best group of people I have ever worked with. I have kept my job for years despite a 50 mile commute because of the people. I love that my people support each other and collaborate because they care about patients. I love that we don't have a culture of complaining. It is very sweet when a patient says they have h

With over 10 years of home health experience I wanted to give them a shot because they were looking for "Home Care Associates" in my county. I had CNA experience, but not certified and not recent. They do not seem to care what an employee's background, training, or competency is and that is because they will send you to any type of client and you are going in blind. You might know if they use a cane or a walker, but you do not know if they are on a vent, have a highly communicable disease, are verbally abusive, have bed bugs in their home, weigh 500 lbs and are a total lift, etc. You are supposed to be prepared for the worst with no heads up and that is unsatisfactory. You are mailed a schedule weekly and even if you say you are not available certain days because of a second job, they schedule you anyway. If you ask for a certain number of hours per week, you may or may not get them. I called the office multiple times trying to talk with my "scheduler" and finally resorted to email and still they could not get it right. Clients don't know you are coming, who you are, and you are told you are not allowed to call clients. IMO that is because if clients knew you were coming they would most likely refuse a visit.The few clients I spoke to said they did not have the same worker most times and did not know who was coming and when. There is no team work. You are cast out there on your own with a name and an address and that is it. The pay is low and you only get a dollar mileag

St. Vincent's Home Health is a home care agency that is managed by Almost Family, Inc and not by the Hospital. New management in place since my time there. Pay is per visit and not hourly or salary for full time field nurses. $40-$50 per routine visit. $68-$90 for admission visit depending on experience. Additional $5-10 on nights or weekends. There is a mandatory 24 hour on call schedule for 1 full week a month in addition to a full work day for 1 RN, 1 LPN, and 1 manager to cover the entire agency/ city/ new admissions after 5 pm - 8 am. It is more than likely that you will have several patients after you finished your day to go across town to see well into the night or wee hours of the morning. Electronic charting with Homecare Homebase tablets. Tablet has GPS system, tracking, and counts down mandatory minimum 30 minute visit time. All charting is due 24-48 hours. Daily assignments done by schedulers in the office. You will know by 6:30 pm of who is to be seen the following day. Do not check before then because your schedule may change. You will need to call the patient the night before and make an appointment time. You will also need to check your device periodically during the day for additions to your schedule assignment as visits may be added. Very little hospice experience in the office, the manager appears stressed and has no time to talk with employees, never responds to e mails either. This nurse has worked in 4 hospices over 21 years and find this hospice to be the most undesirable and poorly run, with little hospice experience in the office and poorly managed pain control in the patients. There seems to be an effort to not change, closed to new ideas, desire to be stuck in the past. I feel I have gone back 20 years. The scheduler overloads the nurses, putting say 5 patients on the schedule, with one patient that takes two hours of driving time. Lots of miles and driving time, huge territories, not uncommon to have one patient 70 miles one way, no credit for the miles, still expected to see 4-6 patients a day. Pain management is poor, The Dr uses lots of Tylenol based products (Norco and Tylenol/Codeine) for long term pain management even with end stage Liver disease. I found many inappropriate use of meds: ie Morphine is used even with stage 3 and 4 Kidney disease. Kidney function is not evaluated before using morphine. Fentanyl seems the drug of choice for long acting pain management, but most not using I never saw nurses doing a med rec or teaching. One heart disease patient was using three anti coagulants. The lowdown of working at chh FOR LHC group
Typical day consists of finishing any documentation left from last night ,check and address email, compare electronic schedule to handwritten schedule, reconcile with schedule, call any doctors for orders. After finishing any personal issues, call first patient and head out. I can expect multiple emails during day which need to be address, several calls for me orders, try to complete first full screen in each pt home, continue pt visits, may be changes pts make to schedules. I must plan an hour at least for each visit, typically 5-6. Head home when completed ,an on at least 20 min At home for each routine visit , longer for any complex visit, may get a few evening emails. A little time to eat and spend time with family, then to sleep and begin over again. This is a true typical day, but many variations occur. I have become much more electronically savvy, am learning of a lot of new validated tests to be used for objective measurement, have become more knowledgeable about meds and cardiac issues The local management is very good and helpful, supervisors answer questions, and help me get work done with very little if any reprimands. I have been very fortunate with my local management team. They have my back Workplace culture is supportive, pt care a priority, never feel pressure to do anything unethical, never feel that $ drive my local office function. Only time you go in the office is for paper work. You have to make a appointment inorder to talk to office staff. You can't put a face to name with the office staff, since you go to the office only maybe once or twice a week. Majority of the clients were very laid back. A few of them were very rude and demanding.

Avoid!!!! I’ve worked for this agency for close to 20 years only to be hit with a huge pay cut in 2020, cut in visit numbers per week, and no pay increases for the foreseeable future! CEO is only in this industry to increase his net worth and increase his company’s stock value! PDGM is reason given for drastic cuts to therapists and therapy services for Medicare patients.

They are gunna get in huge trouble with the state. I don't see the La Crosse Wisconsin Program having a positive future. They lack communication majorly. The interview process was misleading. Worked there for 5 months and never got a name tag when they said they would give you one. Good luck getting a referral bonus.
